Post mastectomy rehabilitation

Taking charge of a mastectomised woman leads us to consider the many aspects that this entails. Lymphedema, for example, is only a small part of a much more complex problem. In addition to the joint component, the muscular strength, we often find ourselves faced with a surgical wound which, adhering to the tissues, modifies their elasticity and sliding capacity. Even radiotherapy causes a situation of harnessing of the involved and neighboring tissues and this, even after some time. The patients also complain of pain in the shoulder, neck, jaw, muscle contractures. A manual fascial and connective tissue treatment can be a valid help since it intervenes on the support and union structures between the various tissues, thickened and inelastic structures, often fibrotic. The band wraps around our body, insinuates itself between the various organs, creates union, support, facilitates the sliding of the tissues. When this fluidity fails, something jams and the whole system will be put in tension. It will be our task to help restore elasticity, reduce edema, recover movement, intervene in postural and asymmetrical problems.

For patients with walking difficulties who prevent coming to the office there is the possibility of using the home service.

The crazy story of an Italian boy

Complex Structure of Oncological and Degenerative Spine Surgery. IRCCS Rizzoli Orthopedic Institute Bologna

In February 2019 Ryan was diagnosed with a recurrence of Ewing's Sarcoma in a well-known hospital in the north, after three years the nightmare had started again, in that hospital my son had entered with his legs, a whole vertebral column albeit affected by a tumor and the desire to fight as he had already done three years before. When I entered the study of Dr. Alessandra Longhi (Director of the Rizzoli chemotherapy department) on June 6, 2019 I was alone because Ryan was unable to bear a transfer as he was completely paralyzed from the chest down, stuffed with painkillers such as methadone and fentanyl in horse doses that didn't give him any relief anyway, he couldn't eat sleep or speak for the pains, I had the resonance with me that after four months of treatment I managed to get that he told me that she had completely disappeared a vertebra, a diagnosis of deep vein thrombosis found by pure chance and in the ears the interview with the doctors of the hospital where he was hospitalized who said to me: Madam resign there is nothing to do it cannot be operated and in any case it is useless to seek to fix the spinal column in all probability will die from this tumor and in any case the bone marrow damage is not recoverable.
I remember very well the meeting with the Doctor, after listening to me, looked at the resonance and sentenced: at the moment I am of no use here it is necessary that Gasbarrini intervenes.
The next day Dr. Alessandro Gasbarrini called me and after a few days he let me know that he would have liked to see me in person. The first meeting with him is one of the many things that I will never forget, he received me after an operation that lasted 10 hours and despite everything, as soon as he left the operating room, he immediately came to me, he listened to me, looked at the resonances and he told me: I operate it, presumably I save it but there is no possibility of recovery since the marrow has changed color, if it ever recovered it will not be my merit but thanks to God. So said it seems a very bad answer but in reality it is managed to give me the peace of mind that however all was not lost. I left him saying: Doctor, I want a miracle from you and I was sure that the man would be able to do it. It was June 11, 2019 and despite all the difficulties that Gasbarrini and his staff encountered for Ryan's transfer on June 14, he was operated on. In those two days that preceded the operation I really saw how hospitals should work, even though the ward was full it seemed that only my son existed, in reality all the patients feel treated like this. Of course I had to talk to many operators and doctors and everyone was trying to prepare me for the fact that I would have to resign myself to paralysis anyway, but I kept telling them that although looking crazy I was convinced that he would recover. On June 15 I was in intensive care with Ryan at one point I looked at his legs suddenly I see that a big toe is moving I ask him to do it again and he repeats the movement. You can't really explain what we felt at the time. Back in the ward, the physiotherapists immediately started to work tirelessly, day after day the movements went out one by one after a week many motor quotas had reappeared. After a month of hospitalization they transferred Ryan to a rehabilitation hospital, but every three weeks we went back to Rizzoli to do chemo cycles, those four days a month were fundamental for my son's motor and emotional recovery not only to finish the treatments tumors but also for the physiotherapy he did there, without taking anything away from the rehabilitation institute, to be honest I have to say that at Rizzoli they set up Ryan for the first time, and always at Rizzoli he walked for the first time. I could write a book on the experience of Ryan at Rizzoli, I have a thousand episodes impressed on the mind that testify the skill and humanity of all the doctors in particular, of course, of Gasbarrini and Longhi, but this was to be a summary of what happened to my son about how in life if you meet the right people and if you really believe in it, a tragedy can turn into a miracle. In conclusion I would simply like to thank all the staff of CVOD, that of chemotherapy, that of intensive care (with infinite affection for Doctor Facchini Fabio whom Ryan affectionately called Doctor Morfina because he always managed to calm him and remove his pain), the physiatrists and the physiotherapists ( in particular Mattia Morri) and finally Doctor Giuseppe Rossi who should have limited himself to embolization instead became Ryan's greatest motivator. Needless to say, I will never thank Dr. Alessandro Gasbarrini who literally gave my son's life back (I often say that I gave birth to him in 2002, but he gave birth to him again in 2019) and Dr. Alessandra Longhi who still today he follows him with a truly incredible dedication.

Testimony collected by Gianni Giardinelli

Never in my darkest nightmares have I imagined being able to see and experience what has happened here in our hospital for three weeks. The nightmare flows, the river gets bigger and bigger. At first some came, then dozens and then hundreds, and now we are no longer doctors, but we have to draw lots and decide who should live and who should be sent home to die, even if all these people paid Italian taxes For all life!
Until two weeks ago, my colleagues and I were atheists; it was normal because we are doctors and we learned that science excludes the presence of God.
I always laughed about my parents going to church.

A 75 year old pastor came to us nine days ago; he was a kind man, he had serious difficulty breathing, but he had a Bible with him and we were struck by the fact that he read it to the death and held it tightly in his hands.
We were all tired, discouraged, mentally and physically exhausted when we had time to listen to him.
Now we have to admit: we as human beings have reached our limits, we cannot do more and more and more people die every day. And we are exhausted, we have two colleagues who have died and others have been infected.
We realized that where man does not arrive we need God, and we started asking him for help when we have a few minutes; we talk to each other and we cannot believe that as wild atheists we now seek our peace every day by asking the Lord to help us so that we can take care of the sick.
Yesterday the seventy-five year old pastor died who, despite having had more than 120 dead here in three weeks and we were all exhausted, destroyed, had managed to bring us a PEACE despite our conditions and difficulties, which we did not hope to find anymore.

The pastor has gone to the Lord and we will soon follow him if he continues like this.
I haven't been home for 6 days, I don't know when I last ate and I'm becoming aware of my worthlessness on earth and I want to take my last breath to help others. I am happy to have returned to God while I am surrounded by the suffering and death of my fellow men. Letter from Julian U. A 38-year-old German doctor who works in a hospital in Lombardy.
